POSITION:   Service Coordinator, Part-Time – 25 hours per week

                        Senior Services

POSITION REQUIREMENTS:

  • Bachelor’s degree in Human Resources, Psychology, Special Education, Social Work or related fields.
  • Must demonstrate past work or educational experience that indicates the ability to learn new tasks.
  • Must be an individual who projects empathy, patience, and compassion through personal communication.
  • Must possess past educational or work experience that demonstrates effective verbal and written communication skills.
  • At least one year of work experience in the field of Audiology or visual impairment is preferred.
  • Must possess past education or work experience that involves some formal presentations such a teaching, public speaking, or in-service.
  • The successful applicant must submit to a criminal history background information check including, but not limited to FBI and ABI record repositories.  Upon offer of employment, the applicant must present a non-refundable fee of $49.00 and appear for scheduled finger printing appointment.

PERFORMANCE RESPONSIBILITIES:

·         Provide vision and hearing screenings.

·         Demonstrate assistive hearing and vision devices.

·         Provide case-management services, on-going service coordination and follow-up.

·         Provide in-service training on vision and hearing loss to agencies where seniors most often visit and are housed.

·         Complete progress reports.

·         Plan home visits.

·         Maintain ongoing contact with center managers where senior programs are housed.

·         Attend required workshops.

·         Complete and submit necessary paperwork, forms, documentation and maintain records of service.

·         Participate in interagency meetings and contact agencies for new referrals.

·         Provide referrals for seniors and family members as needed.

·         Perform necessary networking responsibilities with service providers.

·         Complete all service evaluation related tasks.

·         Perform other duties as assigned.

 

Reports to the Director of Senior Services

 

SALARY:      $26,843 - $35,808                Scale PI Rank 52

                        (260 Days per year – 5 hours per day)

           Direct deposit is required.

                        This is an exempt position, and is not subject to overtime or compensatory

                        provisions of the Fair Labor Standards Act.
