1. Read the screen completely, maybe line by line, so you know what is going to be requested. Turn off Browse mode and go into forms mode. Tab through the form filling in all requested data.
2. Click on Send Button when form completed. You will now be in your email program and using its send key will send the message or put it in the out box to be sent the next time you issue the send command. (note: If your email program does not send immediately make sure you start your email program and send the message as soon as possible.)
3. When you return from the email program turn on Browse mode in your screen reader and tab to the PayPal Button and click. This will take you to PayPal and if you have an account you will enter ID and Password. If you do not have a PayPal account go to the top of the page tab to he amount field and enter the amount to be paid. Then click on continue and complete all the PayPal data for using Credit Cards or other payments.
4. You will be returned to the application screen and you are done so just return to AzCB using the link at the bottom of the page.
Notes:
There are several Affiliates which you can join to be in a group with more focused issues and with extra social activities. You must check the AzCB box if you are not a member for the year you are filling the application out for. If you wish to be in any or all the Affiliates check the box for that group. The amount field is the total of all the boxes checked. This is the amount you will put in the PayPal field.
If you would like to mail in the form and a check use the print option of your Internet Browser after you filled out the form and do not send the email. Put the printed form and your payment in and envelop, stamp, and send.
