Please read the instructions carefully before beginning. Completed hard copy application must be typewritten. Should you choose to submit the application on line, please do so at mrsmcdee@qwest.net, kevinchinn@qwest.net, or www.azcb.org.
See address below for submission of supporting documents. Include the following:
1. Autobiographical Sketch: No more than two (2) typewritten pages containing information about you- your name, address, phone number, date of birth, and e-mail address. Tell us about your personal goals, strengths, weaknesses, hobbies, honors, achievements, etc.. Be sure to list the field or courses of study you are pursuing and explain why you have chosen it. Also tell us about your work experience and extracurricular activities.
2. Certification of legal blindness from an ophthalmologist, optometrist, physician. agency executive serving the blind, or other competent authority.
3. Certified transcripts from the school you are currently attending. Entering freshman and sophomores. please include high school transcripts; graduate students, please include undergraduate transcripts. Unless extenuating circumstances exist, a 3.3 grade point average is required.
4. A recommendation letter from a current or recent instructor.
5. Proof of acceptance from a post-secondary school. Entering or transferring students must submit a notice or letter from the admissions office certifying acceptance or consideration for admission Students continuing at the same school attended during 2006/2007 do not need to submit this document.
Online materials must be electronically submitted no later than April 27, 2007. All other documents not online must be postmarked no later than April 25, 2007 to the following address:
