CASE MANAGER III 
(Full-Time/Unclassified/Full Benefits) 
$15.418 - $21.084 per hour 
The Center for Families and Youth (CFY) in the Human and Social Services Bureau has two (2) openings for full-time, 
permanent, unclassified Case Manager III. The positions perform in-home, outreach counseling services for the Family 
Preservation Program for families whose children are at risk of placement in out-of-home care and are referred to the 
program by the L.A. County Department of Children and Family Services and the L.A. County Probation Department. 
The position reports to the CFY Clinical Director. 
EXAMPLES OF DUTIES: 
·  
Provides in-home counseling/case management visits to participating families and monitors and follows-up on 
client participation and crisis intervention services, as needed; 
·  
Attends Multidisciplinary Case Planning Committee meetings and contributes to the development and 
monitoring of an individualized service plans for each family; 
·  
Observes children and assesses the home environment for child safety risk factors; 
·  
Provides referrals and linkages to parenting training, youth services, mental health services, housing services, 
job training, transportation and other needed services, identifies community resources and pursues new 
referral contacts to meet client needs; 
·  
Serves as liaison and advocate for families participating in Family Preservation and maintains case records 
and required reporting documentation, including, progress notes, monthly reports, service logs, data collection 
and referral forms. 
·  
Attends staff meetings and trainings and assists with events, activities and services of other CFY programs as 
required; 
·  
Performs other duties as assigned. 
MINIMUM QUALIFICATIONS: 
·  
Education a Bachelor’s Degree in Social Work, social services or a closely related field and two years 
experience providing case management or in-home services or experience working in community-based 
programs with diverse populations; 
·  
Excellent verbal and written communication skills; 
·  
Bilingual English/Spanish highly desired.
