Jessie, I don't remember any of my schools requesting any documentation. 
When I was working on my masters through Marshall University, I did not use 
their disability services.  Fortunately, I had no need to.  I was able to 
work with the instructors.  The only issue I had to deal with, and it was a 
small issue, was the need to take the Miller's Analogy Test (MAT).  For some 
strange reason, there were unable to get it in Braille, which was what I 
requested.  I don't know any other way to take that kind of test.  They sent 
a large print copy, and they wanted me to bring a reader with me to the 
testing site.  I told them that was not an acceptable accommodation for that 
type of test.  Marshall required that graduate students take that test 
either before entering their program of study or some time within that first 
semester.  I told them if I couldn't take the test because the requested 
accommodations were not available, I should still be able to register for 
classes for the next semester.  They said they would waive that requirement 
if my grades were high enough.  That is what happened.  I do think they were 
tired of dealing with my request.
Aside from that, my experience with Marshall was great!
