If you are a blind or visually impaired Social Security applicant, beneficiary, recipient or representative payee who has: 1) not made a request for notices and communications to be sent in alternate format or 2) has not received the alternate format of your choice please contact Charlotte Lanvers at 510-644-2555 x 5231 or clanvers at dredf.org by March 15, 2012. SSA provides one of the following alternate formats automatically: audio CD, MS Word on CD, Braille, Large Print (18 point), standard print (12 point) sent with a follow up telephone call, or standard print (12 point) sent via certified mail. If these formats do not work for you, you can request the alternate format of your choice by calling SSA at 1-800-772-1213; SSA is required to issue a decision on your request for non-automatically granted formats within 45 days. Thank you.
