There isn't really a list of the rehab agencies in the United States
The ones I know off the top of my head are as follows
New York State Commission for the Blind and Visually Handicapped
New Jersey Commission for the Blind
Michigan Commission for the Blind
Kentucky Office for the Blind
Utah Division of Services for the Blind and Visually Impaired
Board of Education Services for Blind and Visually Impaired  (Or 
something close to that.)
Department of Adaptive and Rehabiliative Services
New Mexico Commission for the Blind

Also if you type in to google for example services for the Blind in New 
York State you'll get what you are looking for.
