UTAH COUNCIL OF THE BLIND
2008 SCHOLARSHIP APPLICATION DIRECTIONS
The Utah Council of the Blind (UCB) will be awarding several scholarships, made possible by the Poulson Family Scholarship Fund.
This year's scholarships will be for $1,000.00 or more. The scholarships will be presented at the UCB annual state convention, Friday, May 16, 2008. All applications must be received by Thursday, May 1, 2008. (Please advise any and all qualifying students who are blind or visually impaired!
I. Eligibility Requirements for the Poulson Family Scholarship
1. The application and all supporting documentation MUST be received by May 1, 2008. The materials may be either mailed or faxed. Mail to:

2. The applicant must be a resident of the state of Utah.
3. The applicant must have a visual acuity of not greater than 20/70 or a visual field not exceeding 30 degrees or a functional visual impairment of similar degree in the best eye with best correction.
4. The applicant must be a member of the Utah Council of the Blind. (If not currently a member, send a check or money order for $10.00, paid to the order of the UCB (Utah Council of the Blind), to the mailing address above.)
5. Scholarships will be awarded only to students attending institutions of higher education within the state of Utah.
6. Recipients of UCB scholarships will be required to submit a written report outlining the use of scholarship funds. This report will be due by January 1, 2009. (Non-compliance with this provision could mean disqualification for next year's scholarships.)
II. Instructions for the Poulson Family Scholarship
The application and all supporting documentation must be typed or computer printed. (Handwritten applications and documentation will not be accepted.)
The following documents must be submitted for consideration:
1. Completed application.
2. Autobiographical sketch -- No more than two double-spaced typewritten pages containing information about you. You may want to include such particulars as your personal goals, strengths, weaknesses, hobbies, honors, achievements, etc. Be sure to list the field or course of study you are pursuing and explain why you have chosen it.
3. A certified grade transcript from the school you are presently attending or which you most recently attended. (Entering freshmen, please include high school transcripts. New graduate students, please include undergraduate transcripts.)
4. One (1) letter of recommendation, preferrably from a past or current instructor.
5. If you are an entering freshman or graduate student or are a transfer student, submit a printed letter or notification from the school's admissions office corroborating your acceptance to said school.
Note: Interviews may be held following submission of scholarship application and supporting documentation.
